As a courtesy to you, we will submit claims on your behalf for In-Network and most Out-of-Network policies, and assist you in any way we can reasonably do so to get your claims paid. Please be aware that some services may not be covered under your plan, and the balance would be your responsibility. Please contact your insurance company with any questions you may have regarding your coverage.
We ask that you pay the estimated co-pay/co-insurance and/or deductible at the time of service.
